SI 00604: Completion of Form SSA-8000-BK, Application for Supplemental Security Income

A. Introduction

The purpose of the FS-SSA/APP and FS-Referred blocks is to indicate if we completed a Supplemental Nutrition Assistance Program (SNAP) application, formerly called food stamps (FS), or if we referred a claimant to the local SNAP office. We use this information to bill the Food and Nutrition Service (FNS) for the number of SNAP applications we complete.

B. Procedure for FS-SSA/APP and FS-Referred blocks

After you complete question 58, mark one block, if applicable. For question 58, see SI 00604.096.

NOTE: Do not mark the "FS-SSA/APP" block unless you complete a SNAP (Food Stamp) application.
