DI 27530: Reopenings - Completion of Form SSA-831-U5
BASIC (01-12)
The field office (FO) is primarily responsible for completing items 1-14 on the Form SSA-831 Disability Determination and Transmittal. However, the Disability Determination Services (DDS) is responsible for reviewing the FO-completed items on the SSA-831 (or other FO-completed transmittal); and, contacting the FO to resolve any discrepancies such as name, social security number, or type of claim.
If an item can be completed in the same manner for both initial and reopening cases, reference the listed Program Operations Manual System (POMS) section. If the item is not listed below, leave it blank.
After preparing the determination, route electronic cases to the FO. Route paper cases per DI 32005.000 Routing - Initial/Reconsideration/Reopening Cases.
