  3. Choice 1 - “this individual is not currently receiving Social Security payments.”

     Choice 2 - “you recently ordered that we no longer withhold this money.”

     Choice 3 - “this individual has paid the full amount that the court order required.”

  4. dollar amount of last payment

  5. month and year

  6. Use the sentence below only when using choice 1 of fill-in 3:

     “If this individual receives benefits again, we will need a new court order before we can resume withholding money for child support, alimony, or court ordered victim restitution.”
