A. General information on the EDCS Routing Form
The field office (FO) uses the EDCS routing form to send folder documentation (i.e., medical evidence submitted by the claimant and not faxed into the Certified Electronic Folder (CEF), a paper Modular Disability Folder (MDF) associated with the CEF) to the receiving office.
When transferring a CEF, the Field Office (FO) has the option to print the EDCS routing form. If there is documentation to send, the FO
prints the routing from,
attaches it to the documentation, and
mails it to the receiving office.
If you are not sending documentation, there is no need to print the EDCS routing form
NOTE: Any component can print the EDCS Routing Form at any time through the “Print Forms” link in EDCS or eView.
B. Description of the EDCS Routing Form
The EDCS routing form contains the
office address information used to route the case from one office to another;
case identifying information including claimant name, SSN, claim type and claim level;
EDCS folder number sometimes used to search for the case in eView.
NOTE: The EDCS folder number is not a social security number (SSN).
