For a definition of TCAs, see GN 04440.230.
NOTE: Do not return group I and group II deficiencies to the adjudicating component using form SSA-847-U3. Cite all group I and group II deficiencies on an SSA-1774-U5 (Request for Corrective Action).
B. Procedure for completing the SSA-847-U3
1. Field completion for the SSA-847-U3
The Office of Quality Review (OQR) legacy system propagates the following information onto the SSA-847-U3:
claimant’s SSN,
claimant’s name,
claim type,
claim level, and
name of the adjudicating component.
For all other fields, mark the appropriate boxes and fill in the corresponding text, when appropriate.
2. Remarks for the SSA-847-U3
In Remarks, include the following:
rationale for the return;
appropriate references supporting the rationale for the return;
corrective action required (if applicable); and
subsequent folder routing instructions.
3. Disposition of the SSA-847-U3
Make copies of the completed original SSA-847-U3 for paper cases. Place one copy of the form in the paper folder and mail additional copies to the adjudicating component and center for disability, when applicable. For further explanation on distribution of the SSA-847-U3, refer to GN 04440.501B.6.
For certified electronic folders (CEF), the same disability components that currently receive copies of the SSA-847-U3 should also be able to view the SSA-847-U3 in the CEF. Upload the SSA-847-U3 by selecting the [send to EF] block at the OQR legacy system clearance. For further explanation on distribution of the SSA-847-U3, refer to GN 04440.501B.7.
NOTE: The OQR Weekly Deficiency Report only includes TCAs for regulation basis code changes. The weekly report does not indicate other TCAs.For a prior paper modular disability folder (MDF) and a current CEF, return both the prior paper MDF and the CEF to the adjudicating component for corrective action, if necessary.
