A. One Disabled Child—NO DIB
NH's SSN—see GN 01050.165 A.1.
Child's name—Print child's surname and first initial on the reverse of the folder tab, with the surname first.
B. Two or More Disabled Children—NO DIB
Where two or more disabled children are filing on the same NH's SSN and all material is being sent simultaneously to the same office (Office of Disability Operations (ODO) or DDS), prepare one folder for each CDB per A. above. Staple the back flap of one CDB folder to the front flap of the second CDB folder and so on.
C. CDB and DIB Applications Filed for Different Individuals—One SSN
In DIB cases where there is also a CDB application based on the DIB claimant's E/R, prepare separate folders. Prepare the DIB folder as in GN 01050.165 and the CDB folder as in A or B above. If both claims are sent simultaneously to the same office, staple the back flap of the DIB folder to the front flap of the CDB folder. Do not transmit the CDB claim before the DIB claim unless the NH has also filed for a RIB. Cross-refer in remarks section of SSA-831-U5.
D. One Individual Claiming CDB and DIB—Two SSN's
Where an individual files a CDB application on his parent's or grandparent's SSN and a DIB application on his own SSN, transmit the two applications simultaneously in separate folders. Prepare a DIB folder in accordance with GN 01050.165 and the CDB folder in accordance with A. above. In transmitting a combined CDB and DIB claims, staple the back flap of the DIB folder to the front flap of the CDB folder.
