A. Procedure

1. FO assistance

  • Prepare special form letters addressed to the custodian of the records (see GN 00301.195);

  • Enlist the help of third parties (see GN 00301.200);

  • Request the help of other FO's (see GN 00301.205).

2. Claimant authorization

Have claimant sign Form SSA-8510 (Authorization for the Social Security Administration to Obtain Personal Information) to enable SSA to obtain evidence /information on the claimant's behalf.
