A. Use of Form
This form is used as a transmittal to return by mail evidence documents which an individual has submitted in connection with his application for a social security number. It advises him of the processing time for issuance of a card and indicates he should contact his nearest SSA office if he does not receive his card within that time frame.
B. Preparation of Form
Type or stamp the district office address beneath the legend “Social Security Administration” at the top of the form.
Type the applicant's mailing address in the lower left section of the form, adjacent to the dot. The form may be used with a window envelope.
Check the appropriate box(es).
Enter estimated central or local processing time, as appropriate.
