1. Include information per RS 01404.205D.4.; and

2. The following information:

   • location of the home base (station) of the unit to which the individual was assigned for this short-term active duty for training; and

   • location of the place where he/she performed his/her training; and

   • if available, the FEIN of the home base (station of the unit to which the individual was assigned); and

   • the date of the training period.

3. Obtain the additional information needed in bullets 1, 2, and 4 above from the orders the reservist received assigning him/her to a particular local unit.
