B. Requesting instructions

  • This notice is sent by the office that receives a garnishment order (PC or FO) when SSA cannot comply with the order because it is not for child support, alimony, or court ordered victim restitution, or there is not sufficient information to identify the beneficiary (GN 02410.210A. and GN 02410.215A.).

  • Use paragraph 3728A if the order is not for child support and/or alimony.

  • Use paragraph 3728B if the order does not have sufficient information to identify the beneficiary.

  • When paragraph 3728B is used, also use paragraph 3728C, 3728D, 3728E or 3728F as appropriate.

     

Fill-ins:

  1. case/civil action number

  2. beneficiary's full name as it appears on the court order

3728C:

  1. name of person who caused the order to be served

3728D:

  1. name of the representative of the person who caused the order to be served

3728E:

  1. name of person who caused the order to be served

3728F:

  1. name of the representative of the person who caused the order to be served
