- A. Procedure
- Follow these steps for reviewing claims in the totalization module:
- * Review the file for the required applications and evidence listed in GN 01743.205 – GN 01743.220.
* If the person is filing for Australian benefits and the two special Australian applications, the AUS-USA 4 and the Mod (i) are not in file, mail the instructions for completing the application (Part A), the application (Part B), and form Mod (i) to the applicant. Ask them to complete the applications and return them to the Office of International Operations, Totalization Module, P.O. Box 17049, Baltimore, MD 21235-7049.
- * If the person is filing for Australian benefits and the two special Australian applications, the AUS-USA 4 and the Mod (i) are not in file, mail the instructions for completing the application (Part A), the application (Part B), and form Mod (i) to the applicant. Ask them to complete the applications and return them to the Division of International Operations, Totalization Module, P.O. Box 17769, Baltimore, MD 21235-7769.
- NOTE: The Australian application and instructions and form Mod (i) may be printed from the SSA Totalization Forms section of the Totalization Resource Kit.
* If the person is filing for Australian disability benefits and the file does not indicate that the Australian Work Ability Form (form WA) is in file, send that form to the applicant and ask them to complete the form and return it to the Office of International Operations.
- * If the person is filing for Australian disability benefits and the file does not indicate that the Australian Work Ability Form (form WA) is in file, send that form to the applicant and ask them to complete the form and return it to the Division of International Operations.
- NOTE: The Australian Work Ability form may be printed from the SSA Totalization Forms section of the Totalization Resource Kit.
* Check the file for a U.S. Totalization earnings record that can be sent to Centrelink. If one is not in file, request it from OEO, following the instructions in MSOM ICERS 001.001.
- * Check the file for a U.S. Totalization earnings record that can be sent to Centrelink. If one is not in file, request it from OEIO, following the instructions in MSOM ICERS 001.001.
- * When all required material is in file, refer to the guidelines in GN 01744.205 – GN 01744.210 for preparing a claims package for Centrelink.
