  • A. Introduction
  • The application form(s) that an applicant must file will vary depending on the type of benefit claimed (i.e. regular or Totalization) and the country from which benefits are claimed.
  • B. List Of required applications
  • The following chart lists the application forms that must be completed when the applicant is filing in a U.S. field office. Foreign Service Posts should follow the instructions in the subchapters dealing with the specific agreements when determining which application form to take.
