This section describes how to complete amended awards in general.

B. Definition

An amended award corrects or changes a material factor of entitlement of a prior award.

C. Procedure

This chart shows how to complete general amended awards on Form SSA A–101.

Complete items 1 through 8 and item 12

Enter the remark “AMD AWD” or “Amended Award” and reason for amended award in item 11.

If using the folder to process the case, the CAs

  • Circle the corrected item on the initial award

  • Enter “Corrected by amended award (dated (MM/DD/YY)” on the initial award.
