A. Process — Puerto Rico vital records
You can also find information about requesting vital records from Puerto Rico (PR) at the National Center for Health Statistics or http://www.salud.gov.pr/ (Spanish) - see subsection D., below. In addition, you can find EIN information here .
All PR FOs can verify or certify a birth/death/marriage record for a vital records event that occurred in their service areas. FOs should use the e562 to submit their requests. Wait 45 days before following up on either fax or mail requests.
Show claimant's maternal and paternal surnames (e.g., Gonzalez-Gomez) on all requests for birth, marriage and death records.
B. Procedure — e562 requests
All PR FOs will obtain verifications of Puerto Rico vital records from local BVS offices.
Submit requests directly to the FO serving the municipality where the birth/death/marriage occurred. Determine the appropriate FO from DOORS.
For verification requests, include the record for which you are requesting verification.
All requests for information/action must be made via the electronic system e562. The electronic SSA-562 or e562 is an online version of the paper form SSA-562.
When the requesting FO receives the completed request, the adjudicator will update the appropriate system (e.g., MCS, MSSICS, SSNAP, EVID) with the vital records information.
C. Procedure — State sources — mail requests
Make USPS money order payable to Secretary of the Treasury. Personal checks are not accepted. Only the claimant may request his or her birth certificate. Include a signed letter and a copy of an identification card from the claimant along with the appropriate information for the record being requested. With the claimant’s disclosure authorization, SSA offices may use the SSA-L706 to have the request returned directly to SSA (for details, see GN 00302.070).
